ATTENTION STAFF: Incident Report Form

Hey guys! This is Liz. 

Recently, we have been having quite a few issues with members both in and out of the chatbox. Because of this, in some cases it has been difficult for the moderators to decide what action to take. Demi and I have decided that it would be simpler to just make a report to be filled out whenever something occurs. This form can be used by both staff and regular members to report incidents that occur on the site, and it should be PMed to all of the administrators promptly after the incident. Here it is:
